Did
you know that your EMS division has a "Premise History" system?
 "Premise History" is critical
information that you have provided us about an individual.
This information is usually about an individual who lives
alone and has a significant medical condition. We will
supply you with a form that will ask about the individuals
medical condition, doctors, hospital preferences, medical
alarms, contact phone numbers and more.
This information is entered into the
911 computer system. If a 911 call is received from a "Premise
History" address we will be immediately notified
of the information that has been provided to us.
